(Your child CAN NOT attend camp without this form!)
USE THIS FORM IF YOU ARE STAFF, LEADER, VOLUNTEER OR YOUTH
COMPLETELY FILL OUT ONLY THE "CLASS 1 PORTION" (THIS IS THE FRONT AND TOP BACK OF THE FORM). THIS MUST BE UPDATED ANNUALLY & ONLY NEEDS TO BE SIGNED BY A PARENT/GUARDIAN, NO DOCTOR VISIT IS NECESSARY. BE SURE TO KEEP THE ORIGINAL FOR YOUR RECORDS AND MAKE A COPY FOR THE CAMP.  WEBELOS OVERNIGHT REQUIRES A CLASS 1 MEDICAL FORM.

4. Medication Records
IMPORTANT NOTE: ANY MEDICATION(S) BEING TAKEN MUST BE NOTED ON THESE FORMS, WHICH SHOULD THEN BE ATTACHED TO THE "CLASS 1 & 2 MEDICAL FORM". MEDICATION WILL NOT BE DISPENSED WITHOUT THESE FORMS.  Only needed if the Scout will be taking medication while at camp.

5. Emergency Contact Names
You must complete this form and bring it to camp orientation or the first day of camp
